If your doctor has prescribed a different dose, you should ask for further information from your doctor or pharmacist.

Those themes that drew the strongest response were around the helpfulness of people and organisations and the most useful forms of help they provided; the value of the website and its contents; what other services MSGV could offer; the impact of Meniere's disease on people's lives; the positive life changes produced by having Meniere's disease and medication issues. 6.1 Helpfulness of people and organisations. Receiving information was important to people with Meniere's disease. MSGV rated as the most helpful in terms of providing information about symptoms and symptom management. Meniere's Support Group was most helpful with prompt delivery of information, which I can refer to on a `bad day' or when symptoms have changed and articles I overlooked become more relevant to me, for example, selective estrogen receptor modulator. Now the labs need to convince more doctors to order the tests. Pam Sherry, a spokeswoman for Roche's LabCorp, quoted a "ballpark retail" price of $1, 360 -- but said insurers that have contracts with LabCorp will probably pay less. But many policies -- including ones from Aetna, CareFirst BlueCross BlueShield and Cigna HealthCare -- won't pay at all, because the insurers consider use of the testing in clinical practice "experimental and investigational." UnitedHealthcare doesn't have a written policy on cytochrome P450 testing yet, says spokesman Steven Matthews, but the company's plans -- including M.D.-IPA and Optimum Choice -- may approve the testing on a case-by-case basis, if it's prescribed and used properly. Federal workers are guaranteed coverage for medically necessary testing, said Office of Personnel Management spokesman Michael Orenstein, because Federal Employees Health Benefit Plan contracts stipulate that FDA-approved tests may not be considered investigational. But do you really want your health insurance plan to pay for the testing? Cozza posed an interesting question: If your insurer learned that you had a CYP defect . could it decide to require preapproval for every prescription any doctor writes for you? No insurance plans are paying for routine genotyping yet. But the field of pharmacogenetics is young. "My prediction, " said Mrazek, "is that, down the line, pediatricians will do this testing during a child's infancy." But first the labs know they have some convincing to do. BJM June 7, 2003; 326: Original investigation, first author Nicola J Cooper, University of Leicester, UK. bmj cgi content full 326 7401 1235 An accompanying commentary pp 1249-40 by Lucy Hansen, University of Edinburgh ; concerned the need to determine who benefits most from flu treatments: No studies have compared the response to treatment in vaccinated vs unvaccinated elderly residential patients. One study of vaccinated patients reported a 92% relative reduction. We often forget that the vaccine is only70% effective, and has only short term benefits. "Rather than neuraminidase inhibitors being an alternative to vaccination, they might be an additional treatment in high risk groups, particularly during epidemics or local outbreaks." Comment: I believe there may be a limited application of these drugs. Oral Tamiflu oseltamivir ; would seem to be more advantageous. It is easier to take and more acceptable to patients. Choosing individuals for whom the drugs are indicated would be a clinical-judgment call, depending largely on cost, personal preference, and likelihood of severe complications from flu. This is another study citing misleading relative reductions in risk rather than absolute reductions. The 2003 PDR provides additional information: Gives no indication for use of zanamivir for prophylaxis. Prophylactic oseltamivir given for up to 42 days in community outbreaks reduced infection incidence from 4.8% to 1.2%. Given in nursing home residents, reduced infection rate from 4.2% to 0.4%. The majority of these had received vaccine. ; For household contacts, incidence reduced from 10% to 1%. The number needed to treat to prevent one person from getting flu varied from 11 to 28. The results cited in the article are tenuous, especially those citing effects of the drugs in preventing complications which result in antibiotic use. Beating Osteoporosis Naturally By Steven Coward, ND Asheville Citizen-Times May 2003 Osteoporosis, characterized by reduced bone density and increased fracture risk, is arguably the greatest health concern of postmenopausal women. As many as 25 million Americans are thought to have osteoprosis, and 430, 000 per year will facture a bone because of it. More women die from osteoporosis than from breast cancer, uterine cancer, and ovarian cancer combined. Hormone replacement therapy with estrogen has long been the conventional treatment, and newer drugs like Fosamax and Evisra are being used more and more. The problems with these are that they have undesirable side effects and they fail to address the basic natural processes that govern bone health. Fortunately, it is easy to keep your bones healthy and strong without drugs. Simply work them and feed them! Weight-bearing exercise is the cornerstone of good bone health. Our skeletons adapt to the demands put upon them. If we work them, they will get, and stay, strong. Consider a 1994 study in the New England Journal of Medicine that put a group of frail elderly women on a twice-per-week progressive strength training program for one year. Some of these people were in their nineties! Nevertheless, their bone density increased 1% on average. A control group that did not engage in strength training lost more than 2% of its bone density on average, typical for postmenopausal women. Incidentally, the group that did strength training saw improvement in balance, which reduces fracture risk, and a 27% increase in overall activity. How are those for side effects. These elders not only began to get their bones back, but their lives too! If this group could do it, so can you. Many other studies have replicated these findings, and others have proved the converse true. Healthy volunteers assigned to twelve weeks of bed rest, for example, lost almost 4% of their bone density at the hip. Use it or lose it! How do we feed our bones to keep them healthy? Everyone knows calcium is important. I prefer food sources because they contain a host of other nutrients in addition to calcium. Our bones are made up of much more than just calcium, and we need to get balanced nutrition to remain healthy. Cheese, yogurt and milk are high in calcium, and organic is preferable to avoid traces of artificial hormones and antibiotics. For those who don't tolerate milk well, there are vegetable sources. Spinach is a well-known source, but turnip greens, collard greens, and rhubarb are even richer. Lambs quarters, broccoli, and tofu are other good sources. In order to absorb our calcium, we need vitamin D. Our bodies manufacture vitamin D when our skin is exposed to sunlight. We can get all we need with 30% of our skin exposed to sunlight for 30 minutes per day without sunscreen. One teaspoon of cod liver oil gives a full day's supply, as does four ounces of sardines or mackerel. Other fish and mushrooms are good sources as well.

Opinion paper, which states that second-generation antipsychotics differ in their diabetes risk profiles. These findings are in direct conflict with the FDA's recent class labeling language. Despite an increasingly competitive environment, we believe the product, together with Symbyax, still has sales growth potential in the U.S. We expect U.S. sales to benefit from the recent approval of a bipolar maintenance indication and the anticipated near-term approval of Zyprexa IntraMuscular. Sales outside the U.S. increased 42 percent, to $1.64 billion. Excluding the impact of exchange rates, our sales outside the U.S. grew 26 percent. The strong international sales growth of Zyprexa was primarily driven by increased unit volume attributable to the bipolar mania indication and the ongoing conversion from typical to atypical antipsychotics and, to a lesser extent, the impact of exchange rates. Zyprexa recorded strong growth in several key markets, including several major European Union countries and in Japan. We expect continued strong overseas growth of the product in 2004. Zyprexa recently received U.S. and European approvals for bipolar maintenance. Zyprexa IntraMuscular has recently been launched in Australia, Canada, and Europe, and we currently expect U.S. approval in the first half of 2004. Diabetes care products, composed primarily of Humulin, biosynthetic human insulin; Humalog, our insulin analog; and Actos, an oral agent for the treatment of type 2 diabetes, had aggregate worldwide revenues of $2.57 billion in 2003, an increase of 12 percent. Diabetes care revenues in the U.S. increased 10 percent, to $1.57 billion. Diabetes care revenues outside the U.S. increased 17 percent, to $1.00 billion. Humulin had worldwide sales of $1.06 billion, an increase of 6 percent. Humulin sales in the U.S. decreased 2 percent, to $507.5 million. Humulin sales outside the U.S. increased 13 percent, to $552.9 million. Humalog became a billion-dollar product in 2003 with worldwide sales of $1.02 billion, an increase of 22 percent. Humalog sales in the U.S. increased 25 percent, to $658.6 million. Humalog sales outside the U.S. increased 19 percent, to $362.7 million. In 2004, we expect our worldwide insulin franchise to have little or no growth primarily due to expected continued competitive pressure on prescription volume. Actos revenues, the majority of which represent service revenues from a copromotion agreement in the U.S. with Takeda Pharmaceuticals North America Takeda ; , increased 10 percent in 2003, to $431.2 million. Actos is manufactured by Takeda Chemical Industries, Ltd., and sold in the U.S. by Takeda. Gemzar became a billion-dollar product in 2003 with worldwide sales of $1.02 billion, an increase of 17 percent. Sales in the U.S. increased 8 percent, to $522.4 million. Sales outside the U.S. increased 27 percent, to $499.3 million. We recently submitted Gemzar in the U.S. for the treatment of late-stage metastatic breast cancer.
