This compound is structurally related to the Didemnins previously isolated from various species of a Caribbean marine tunicate of the genus Trididemnin. In vitro studies have shown that Aplidine interferes with the cell cycle progression at the G1 phase of the cell cycle. In experimental animals, Aplidine mediated an in vivo inhibitory effect in a number of tumor cell types. In humans, Aplidine is currently in Phase I clinical trials. Aiming to predict the hematotoxicity of Aplidine in humans, samples from human bone marrow BM ; and cord blood CB ; were in vitro exposed to increasing concentrations of the drug and then assayed for the clonogenic ability of myeloid CFU-GM ; , erythroid BFU-E ; , megakaryocitic CFU-Meg ; and pluripotent CFU-Mix ; hematopoietic progenitors. In experiments conducted with BM samples, cells were exposed for 1h, 24h, 5d and 1h daily for 5d. Data derived from these studies showed that CFU-Meg IC50: 0.31-0.06 M ; and CFU-Mix IC50: 0.49-0.05 M ; were the most sensitive hematopoietic progenitors, compared to CFU-GM IC50: 1.18-0.12 M ; and BFU-E IC50: 0.960.08M ; progenitors. In subsequent experiments we investigated whether Aplidine hematotoxic predictions based on BM cultures were reproduced when a more readily available source of human hematopoietic cells, cord blood cells, were used in experiments involving 24h exposures. Although hematopoietic progenitors derived from BM were generally more sensitive than those derived from CB, differences on the IC-50, IC-70 and IC-90 varied within a relatively small range of 1.6-6.2 fold. Moreover, data obtained from CB cultures confirmed the observation made in BM assays indicating that the megacaryocyte CFU-Meg ; and the multipotent CFU-Mix ; progenitors were the most sensitive to Aplidine. Regardless the origin of the hematopoietic progenitors BM or CB ; the toxicity of Aplidine was lower than that observed in tumoral cell lines IC50: 0.0002-0.027 ; . Significantly, plasma concentrations in humans receiving maximun tolerated doses in phase I clinical trials were below the IC50 for hematopoietic progenitors maximum 0.0034 M ; and no myelotoxicity was observed to date. Unfortunately, there is no "magic bullet" for patients with CRPS. When a CRPS patient presents for treatment, most authorities generally still recommend a sympathetic nerve block to assess if the patient has SMP or SIP ; . If the patient is found to have SMP, then there is a possibility that s he may experience significant relief after a series of these blocks and, indeed, for a minority of patients may be curative. However, patients and physicians should be warned not to be disappointed if a trial of 1 or nerve blocks indicates that this therapy does not work for them. Each patient is different in terms of response to nerve blocks and medications. Though no treatment has been shown to improve all patients with CRPS, the good news is that there is a long list of treatments that experience has shown to ameliorate the pain and improve quality of life for many patients. It is the responsibility of each pain provider to become familiar with the wide-range of treatments reported to help patients with CRPS, not just perform invasive procedures, which will only benefit few patients.
