Completion of the work, either indirectly by delaying a succeeding operation, or directly if it is the final operation remaining. The Contractor is required by Section 108 to submit a proposed progress work schedule to the Field Engineer. The progress work schedule should indicate the chronological order of operations which the Contractor tentatively expects to follow. This information should assist the Field Engineer in determining the current controlling operation. Work included in the contract may include a group or groups of work items that do not require prior construction to prosecute and that will not delay any other items if they are not constructed. Construction of these items will be considered as non-controlling operations if they are constructed concurrently with controlling operations. If such items of work are simply left until all other work is completed, then the construction of such items would be a controlling operation. 3. Charging of Working Days The District Engineer will issue a "Notice to Proceed" DOT Form No. 258, stating the date upon which work is to proceed. Charging of working days will begin on the date stated in the Notice to Proceed. PATIENTS From January 1, 2000, to June 1, 2004, 28 patients were contacted, 24 were eligible, and 20 gave informed consent and were included Figure 2 ; .
